Please complete the questionnaires below 6 months after rehabilitation from your surgery. It may seem like a lot of questions, but they are often simple and easy to carry out. You may want to take a couple of days to complete them.

The information you submit will be used in one of two ways:

1. To help you and your healthcare professional agree the right care plan for you;
2. To help us get feedback on your recovery to help us improve our future services.

Your clinical team will be able to see your answers once you complete the questionnaire, however as this is a new way to collect this information your team may not always be able to respond in the early stages of the programme. Therefore, if you have a medical emergency or need to discuss any pressing health concerns, please contact either the healthcare professional who is managing your care, your GP or NHS 24 on 111.
