Musculoskeletal Services - Patient Reported Outcome Measures (PROMs)

Patient Reported Outcome Measures (PROMs) are health questionnaires that are designed to capture how you are feeling from your own perspective. 

As part of an ongoing initiative in Cardiff and Vale, we are asking patients to complete PROMs before and after treatments so that your clinical team can better understand your quality of life in relation to your condition.

Your responses will be used to monitor changes in your symptoms and quality of life. This can help you and your clinical team to make informed decisions about what treatment is most suitable for your condition, needs and circumstances.

Before attending your clinic appointment, please could you register and complete your PROMs assessment (if required) on the My Clinical Outcomes website using the button below.

When prompted, please register with your Name, Date of Birth and NHS Number. Your NHS number can be found at the top of your appointment letter.

As part of the registration process you will also be asked to choose your clinical condition and associated treatments from the options provided, these are:

  • Hospital / Clinic: Enter your local hospital "CRI", "UHW" or "Barry"
  • Consultant: Enter “MSK consultant”
  • Medical speciality: Enter “Musculoskeletal (MSK) services”
  • Problem area – select the part of your body that’s a problem
Once registered, you will be prompted to complete your first online PROMs assessment (if required). A reminder email will be sent to you when you are next required to log in and complete a new assessment, this is important to provide information about your ‘health status’ to your clinical team.

If you are unable to fill in the questionnaire, you may be asked to complete this at your clinic appointment.
