The Occupational Therapy team work on Medical, Care of the Elderly and Spinal Injury wards at the University Hospital Llandough.
We work with individuals with diverse medical conditions including chronic conditions, acute physical changes and those who require end-of-life care. We assess people to help prevent a decline in health and people’s ability to conduct everyday living activities to increase independence and to reintroduce daily routines and roles.
We are able to refer people to appropriate services to provide ongoing rehabilitation and support following discharge. We work with the individual, family members and carers to coordinate discharge.
Occupational Therapists also work in the frailty team on the Medical Emergency Assessment Unit to prevent hospital admission and decline in people’s ability to conduct activities of daily living through early assessments, intervention and ensuring an effective treatment plan is in place.
Referrals: Referrals are made to the team by multi-disciplinary team members when an individual is an inpatient within University Hospital in Llandough.
